First Visit At Wichita Family Dental

During your first visit to Wichita Family Dental, you will be greeted by our welcoming staff who will offer you a tour of our facilities. You will receive a menu of comfort items to choose from, including iPads, headphones, warm blankets and face towels, and neck pillows. Our friendly team members will make sure you feel right at home. Your first appointment will include a comprehensive examination and a review of your treatment options. Most often, treatment can be started or completed the same day as the consultation.

Please assist us by providing the following information at the time of your consultation:

  • X-rays from by a previous dentist, if applicable.
  • A list of medications you are taking.
  • Your completed patient information forms.
  • If you have dental insurance, bring your insurance card. This will save time and allow us to help you process any claims.

 

You may also arrive 15 minutes before your appointment to fill out your patient information forms.

*IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves, and joints, etc.).

X-RAYS

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.
